Even though the incidence of gastric cancer is declining worldwide, it still presents high mortality rates. Notwithstanding the therapeutic improvement of chemo- and radiotherapy, surgery still plays the main therapeutic role. Thus, it is of fundamental importance to the surgeon to know of all the standpoints in surgical treatment of this neoplasm, such as curability and resecability criteria, infiltration of resection margins, extended loco-regional lymphadenectomy, en-bloc removal of involved contiguous structures, and, last but not least, the choice of the reconstructive procedure after resection.
