Introduction: The aim of our survey was to verify, in a group of urban and extra urban transport workers, alcohol use\abuse and drug addiction. Subjects materials and methods: 511 bus drivers participated to our study (mean age 41,6 years 10,01, mean working-age 14 years ± 10,31). CAGE test and a blood sample (CDT, GT, MCV, ALT, AST) were used in order to evaluate alcohol use\abuse. The sample was divided into 5 age classes and 4 categories of alcohol consumption (teetotaler, occasional, moderate and habitual consumer). For drug addiction, first level medical assessments were made: physical examination and toxicological-analytic test on an extemporaneous urine sample in agreement with guidelines. Results and discussion: 3 workers (2 to metabolites of THC and 1 to cocaine) were positive to the first level toxicological test. The confirmatory test reduced the positivity to 2. Correlation between MCV and the attitude to drink appeared significant (R2=0,79). CDT is the parameter with the most sensitivity and specificity that alters the value also in the occasional consumers. Then it can be stated that MCV is the more useful in a moderate consumers population and CDT in a non moderate one. CDT has been correlate also to other parameters: AST and ALT (R2=0,83) and GT (R2=0,62).
