OBJECTIVES. The purpose of the study is to show the range of surgical options in pre-prosthetic oral surgery. MATERIALS AND METHODS. The authors report a difficult case where an upper jaw bone was reconstructed by an iliac crest bone graft describing all treatment steps from the beginning to final prosthetic rehabilitation. RESULTS. The 5-years clinical and radiological follow-up showed that it is possible to achieve a stable and reliable implant-supported dental rehabilitation. CONCLUSIONS. Adequate surgical techniques togheter with autologous bone grafts allow for a good implant rehabilitation with successful longterm results even in extreme atrophy of the maxillary bone.
