An MRI-pathological correlation was obtained in 15 disc herniations from 15 patients undergoing surgery. The study showed that when peridiscal enhancement was evident on MRI, granulation tissue with macrophagic cells was present histopathologically. Disc fragment infiltration by this tissue appeared as diffuse enhancement of the disc itself on MRI. High signal on T2-weighted images was related to hyperhydration of the fragment. These data indicate that high signal on T2-weighting and peridiscal enhancement are features typical of the first phases of disc herniation.
