The history of pancreas transplantation has been shaped mostly by improvements in surgical technique and immunosuppression. Robotic pancreas transplantation is perhaps the only true innovation in surgical techniques that have been introduced in the last two decades. Being a minimally invasive procedure, robotic pancreas transplantation can reduce incidence and severity of surgical complications and improve clinical results. First performed in Pisa, Italy, on September 27, 2010, robotic pancreas transplantation has been so far reported by only two Institutions worldwide. These preliminary experiences have shown that robotic pancreas transplantation is feasible, under appropriate conditions, and can result in improved outcomes, especially in terms of fewer wound and surgical site complications. These advantages might be more relevant in obese patients. According to the IDEAL stages of surgical innovation, robotic pancreas transplantation is still in stage 1 (idea) and therefore requires further development and assessment.
